Arrangement Form

After notifying the funeral home at 609.971.6800 that a death has occurred; Use this form to provide us with necessary information which will be used for making the funeral arrangements and filling out the Death Certificate. Once this form is submitted, a representative from the funeral home will be back in contact with you.  You will see a confirmation of successful submission on screen, if not, review form for required missing information highlighted in red and resubmit.

Required Permissions granted to Layton's home for funerals

If you selected a funeral that may require embalming such as a funeral with viewing, you may have to pay for embalming. You do not have to pay for embalming you did not approve if you selected arrangements such as a direct cremation or immediate burial unless the embalming is required by the State of New Jersey.
